1. A 12 year old boy known asthmatic boy
presented to the A&E with sudden onset of severe dyspnoea and
wheezing. On examination, he was too
breathless to talk and respiratory rate is 60/min. There were intercostal and subcostal recessions and nasal
flaring. His peak flow when measured
was 45% predicted value.
2. A 5 year
old boy presented to his GP because the parents were concerned with his poor
weight gain and recurrent chest infections.
His birthweight was at 25th centile but now is only at 4th
centile. Further birth history revealed
that the baby failed to pass meconium in the first few days of life. On examination, the child has hyperinflation
of the chest and finger clubbing.
3. A 1 year
old toddler was presented to his GP with a dry cough and increasing
breathlessness. In the history, parents
revealed that the toddler has had a runny nose a few days ago, and he has been
losing her appetite. On examination,
the findings were tachypnoea, subcostal and intercostals recession and fine
crackers on auscultation.
4. A 6 year
old boy presented to the A&E with acute onset of painful throat and a high
fever of 38.9 degrees. He was unable to
speak or to swallow and was drooling saliva.
The child was sitting immobile, upright and with an open mouth.
5. A 3 year
old boy presented to his GP with recurrent coughing for the past 3 months. On examination, there was dull percussion
note on the right lower lobe and decreased air entry in the same area. Chest x-ray shows consolidation at the right
lower lobe area.
